Community Hospice of Virginia provides hospice care for adults in homes, assisted living facilities, nursing homes, or wherever the patient resides.
Description:
Community Hospice of Virginia service areas include the City of Alexandria, Fairfax City, the City of Falls Church, and the counties of Arlington, Fairfax, Loudoun, and Prince William.
Our focus is on the patient and the family. We treat each patient and the entire family with a holistic philosophy of care, providing support to all involved. Our interdisciplinary team meets regularly to coordinate the critically important task of managing the illness, the symptoms, and the psycho-social and spiritual issues of the patient.
