Our mission is to provide integrated, comprehensive and client-centered primary, acute and long-term health care and social services support to frail older adults. We value caring, compassion, concern, competence, collaboration and creativity. PACE is the only health plan that specializes in the holistic care of nursing home eligible older adults in Vermont.
Description:
PACE is an integrated, all-inclusive provider of services for nursing home eligible individuals living in Chittenden County and the two towns of Grand Isle and South Hero in Grand Isle County. We also have a Center in Rutland serving all of Rutland County and the towns of Dorset, Manchester, and Rupert in Bennington County. PACE is an acronym for "Program of All-Inclusive Care for the Elderly." It is a permanent provider program under the Federal Medicare and Medicaid Services. PACE Vermont is the first "rural" PACE program in the country. Eligible individuals receive both their health insurance plus all health and social care services from the PACE program. Our Center is located on the Fanny Allen campus in Colchester. The Center "houses" the primary care physician office, a nursing infirmary/clinic, out-patient physical and occupational therapies, day-long recreational and therapeutic activities, as well as other allied health care services such as social work and dietician services.
