Description:
Asthma Network of West Michigan (ANWM) performs two different but complementary functions: community education and outreach and in-home asthma education and case management. Community education takes place via regularly scheduled school, medical and allied professional conferences; we have reached over 40,000 individuals through community outreach since 1996. ANWM uses specially trained physicians, nurses and respiratory therapists to accomplish this. Last year, ANWM educators/case managers conducted 1,672 home visits.
History:
Established in 1994, Asthma Network is a consortium of individuals from area health care institutions and related agencies working together to combat severe asthma, reduce hospitalizations, and eradicate asthma deaths.
ANWM has been designated as one of six national model asthma programs; awarded the National Exemplary Award (2006) and the National Environmental Leadership Award (2008) by the EPA; and cited by the Centers for Disease Control and Prevention for its innovation. We were the first program in Michigan to offer an intensive case management component; target a low-income population, and (nationally) obtain reimbursement for home-based asthma case management.
