In-House Hospice Solution is committed to providing palliative and supportive care to individuals suffering from the direct effects of terminal illness. The hospice team strives to meet the physical, emotional, spiritual and social needs of patients and family members who share the emotional trauma of such an illness.
Description:
In-House Hospice Solutions dedicates a team of professionals to bring quality of life experiences to the final stages of the disease process for residents in an alternative living setting. This includes active medical management by hospice physicians who have practiced in Michigan facilities, CNA's, MSW's, IFOC certified Chaplains and trained volunteers.
Each dedicated team provides 5-7 visits per week with additional hours is needed. Through the hospice benefit, a variety of therapies and equipment can be obtained if medically necessary to keep the patient comfortable. Each family receives a weekly update from the Hospice RN Case Manager and/or the Hospice Social Worker.
These professionals make it part of their care to actively participate in care planning and conferences with the facility staff and physicians to keep lines of communication open. In-House Hospice Solutions is available to patients, families and staff of our partner facilities 24 hours/day, 7days/week for consultation, questions, in-service training and grief support.
In-House Hospice Solutions has developed the following SPECIAL PROGRAMS to complement the TEAM of hospice professionals assigned to each building which include; MASSAGE THERAPY, VENDOR MANAGEMENT, PET THERAPY, MUSIC THERAPY, AND FOREIGN LANGUAGE INTERPRETERS.
History:
In 1997, In-House Hospice Solutions developed a unique program designed to meet the needs of the hospice patient residing in a nursing facility. The program is unique due to numerous regulations that must be followed by all nursing homes to ensure the rights, privacy and dignity of all residents. In-House Hospice Solutions personnel are trained in these regulations and partner with facility staff to enhance the end-of-life care provided for their residents and families. In-House Hospice Solutions services a number of counties out of three different offices in Michigan located in Southfield, Kalamazoo, and Grand Rapids.
