Heartland Hospice - Grand Rapids

The mission of Heartland Hospice is to provide quality hospice care to patients with limited life expectancy, as well as, support to their caregivers.
The quality of life for those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial and spiritual needs of the patients and caregivers.

Description:
Hospice care is provided by an interdisciplinary team utilizing an approach to the care delivered to patients either in their home or extended care facility, working cooperatively with area health care providers and other community resources.
Heartland strives to provide care to all who are eligible and need hospice care irrespective of ability to pay.
Although our agency works out of Grand Rapids, we serve patients in eight different counties in West Michigan, including Montcalm, Ionia, Ottawa, Newaygo, Mecosta, Kent, Muskegon, and Oceana counties.

History:
Hospice is a philosophy of care which originated in 1967 in England. Dr. Cecily Saunders opened a hospice to tailor the needs of terminally ill patients and their families and called it St. Christopher's Hospice. The hospice philosophy came to the United States in 1970 and has spread since then.
Our particular agency, Heartland Hospice, is a wholly owned subsidiary of HCR Manor Care, which is located in Toledo, Ohio. HCR Manor Care also owns Skilled Nursing Facilities and Home Care Agencies. The Grand Rapids Hospice office was founded in 1998.
