Description:
Sacred Heart Hospice is affiliated with Sacred Heart Hospital for seamless continuity of care. It is comprised of experienced hospice professionals with 24 hour availability to address care needs. Sacred Heart Hospice offers physical, psychosocial and spiritual care in the patient's home, Assisted Living Facility, Skilled Nursing Facility or Hospital.

Miscellaneous Information

Here are our special events...
	We have a Memorial Service each year where we remember those who have died. The service is followed by a social for fellowship. We also offer Bereavement Camps for children who have lost loved ones. This was held twice in 2008. Dates for 2009 to be announced.
