To provide quality hospice care to individuals, their families and caregivers with "End of Life" care and support. Affinity Hospice also provides grief and bereavement support, education, support groups, as well as 1:1 counseling. Affinity Hospice is available to all community agencies, schools and businesses.

Description:
Honored to be serving patients and families in the Counties of Worcester, Middlesex, Bristol, Plymouth, Essex and Norfolk since 2005.
Affinity Hospice of Life, provides hospice care within a team approach. An individual with a "Life Threatening Illness" that qualifies for hospice care receives the services of a medical team. Services are followed by the patient's physician, the agency's medical director, a nurse manager, social worker, chaplain, bereavement counselor, personal care attendant, volunteers and other services required to provide palliative care and comfort at end of life. Services are provided within the patient's home, within assisted living residents, and in nursing care facilities.
Volunteers will be trained to provide companionship and respite support to patients and their caregivers.

History:
Affinity Hospice is a certified hospice agency. We have been serving patients and their families since 2005. Our hospice has grown to serve patients in 6 states to include Arizona, Nevada, Utah, Michigan and Colorado.
