Description:
Hospice provides multi-disciplinary care as a way to help patients deal with illness and to achieve death with dignity. Care is provided in the comfortable and familiar surroundings of the patient's home, with the care and companionship of family and friends close at hand. Inpatient and outpatient facilities are available to supplement home care when needed. Services are provided by an interdisciplinary team of professionals. Under the direction of the Medical Director, hospice team members use their expertise to control pain and other symptoms of terminal. The Hospice Team of nurses, social workers, chaplain, home health aides and volunteers is available to patients.
History:
Hospice of Boston and Greater Brockton has been providing adult and pediatric hospice and palliative care services to Boston and Greater Brockton surrounding communities since 1979. Hospice has been providing a high level of care to terminally ill patients residing in their home or nursing homes for over 25 years by focusing on the patients' comfort - physical, psychosocial, spiritual and social - as the primary goal in care.
