Description:
PACE is an integrated, all-inclusive provider of services for nursing home eligible individuals living in Chittenden County and the two towns of Grand Isle and South Hero in Grand Isle County. PACE is an acronym for "Program of All-Inclusive Care for the Elderly." It is a permanent provider program under the Federal Medicare and Medicaid Services. PACE Vermont is the first "rural" PACE program in the country. Eligible individuals receive both their health insurance plus all health and social care services from the PACE program. Our Center is located on the Fanny Allen campus in Colchester. The Center "houses" the primary care physician office, a nursing infirmary/clinic, out-patient physical and occupational therapies, day-long recreational and therapeutic activities, as well as other allied health care services such as social work and dietician services.
