A not for profit agency that helps Veterans, Women with Children "Break the cycle of homelessness."
Description:
We are a restorative pathway for individual seeking to get out of poverty and homelessness. We provide an intensive assessment that prioritizes the issues that have lead to a person becoming homeless. They sign a Individualize Planned Program agreement with us agreeing to work on the issues. We offer counseling, "accountability" case management sessions weekly with individual to assist them with their journey. We connect them with other agencies in the community to get the services they need to be lifted out of homelessness ( housing referrals/placement, job readiness skills/job referrals) and we provide transportation. We have a 6 month follow up once they leave to make sure they are still working their plan and on their way to making a permanent transition into a life based upon self sufficiency.
History:
Our organization has been in business serving the homeless in our community for 16 years. We are the program that individual come to when they are tired of living a non productive life sytle and truly want to break out of homelessness.
