Applegate HomeCare & Hospice is a locally owned and operated agency with branch offices throughout the State of Utah. We offer excellent rehabilitation, nursing and hospice services. Our personalized approach allows services to be individually tailored to meet the needs of our clients and their families. Our offices are located in:
