Houston Hospice
Mission Statement: Houston Hospice provides uncompromising, compassionate end-of-life care to patients and families in our community. Through the efforts of a specially trained and highly skilled interdisciplinary team of health care professionals and volunteers, Houston Hospice patients and their families are guided through the process of illness, by receiving clinical care, psychosocial support, spiritual guidance, and volunteer assistance.
Description:
Hospice care is provided in a home, or residential facility such as a nursing home, with the Hospice interdisciplinary team of healthcare professionals and volunteers providing services and support. Houston Hospice serves all or part of Harris, Waller, Fort Bend, Montgomery, Wharton, Matagorda, Austin, Colorado, Brazoria and Jackson counties, through its main office site and three alternate delivery sites in West Houston, North Houston and in El Campo, TX. Since its inception, Houston Hospice has provided care and support to over 10,000 patients and many times that number of family members. Houston Hospice has relationships with a number of managed care organizations and is licensed by Medicare and Medicaid. Furthermore, the agency never denies treatment based on a patient's ability to pay for services. Houston Hospice is a member of the Texas Non-Profit Hospice Alliance and the Texas New Mexico Hospice Organization. Houston Hospice is accredited by Community Health Accreditation Program, Inc. (CHAP). The Community Health Accreditation Program, Inc. (CHAP) is an independent non-profit accrediting body which was established in 1965. CHAP accreditation publicly certifies that an organization has voluntarily met the highest standards of excellence for home and/or community-based health care. CHAP accreditation makes a statement about our commitment to excellence and doing things right. It states that we go above and beyond.
History:
As Houston's largest, oldest, nonprofit organization, Houston Hospice has been providing end-of-life care to Houston and surrounding communities since 1980. The agency provides compassionate, dignified, nonprofit services to patients and families facing the challenge of terminal illness in its last stages.
