Description:
Evercare Hospice and Palliative Care provides hospice services to patients who have received a terminal diagnosis. These services are individualized to address the patients needs. We serve patients who reside in private homes, skilled nursing facilities, or other facility. Hospice services are delivered by a team comprised of both clinical and non-clinical staff as well as unpaid volunteers.
