Serve the uninsured low income medical and dental needs of the community.
Description:
The medical clinic serves the entire La Porte Co. except Michigan City. Patients must qualify first for our services. Proof of household income and address is verified. Patients are seen on a sliding fee scale.
The Dental clinic serves all of La Porte Co. Sliding fee scale for those without insurance and Medicaid is accepted.
Open 5 days a week, no weekends. We are also open on Wed. evenings.
History:
Opened 15 years ago to serve this population. Supported by the La Porte Hospital and Health System, La Porte Hospital Foundation, local foundations, and the Indiana State Dept. of Health.
We are expanding in 2009!! More room and new services will be offered!
