The mission of Heartland Hospice is to provide quality hospice care to patients with limited life expectancy, as well as, support to their caregivers.
The quality of life for those we serve is enhanced through palliative and specialized supportive care designed to meet the physical, psychosocial, social, and spiritual needs of our patients and caregivers.
Heartland Hospice strives to provide care to all who are eligible and need hospice care irrespective of ability to pay.

Admission and treatment is delivered without discrimination of race, color, religion, age, sex, marital status, contagious diseases or sexual orientation.
Hospice care is provided by an interdisciplinary team that includes: Medical Director, Attending Physician, Registered Nurses, Therapists, Pharmacists, Certified Nurses Aides, Social Workers, Spiritual Counselors, Bereavement Counselors and Volunteers.
We help preserve our patients' dignity, show them the respect they richly deserve, and provide caring compassion by making these Heartland Caring Promises, and by working hard to keep them: We promise to care for the mind, body and spirit. We promise to provide care that is comforting and compassionate. We promise to respect patient and family choices. We promise to unite with community partners in care. We promise to be accepting and supportive of patients wherever they are in life's journey.
Hospice isn't a place. It's a philosophy of care. Heartland Hospice is caregiving tailored to fit the patient's and family's unique situation.
Our caregivers help manage pain and symptoms, enabling our patients to live their lives as fully and comfortably as possible. Social workers and spiritual counselors create a comforting environment in which patients and families are able to share their thoughts, hopes, and concerns. Special bereavement programs provide emotional support to patients and families who need help coping. And volunteers help to provide the extra love and care our patients and families need at this time in their life.
The Heartland Hospice team is committed to supporting patients' and families' goals, and encourages families to be involved in all care decisions. Our focus is on sustaining quality of life be encouraging personal choices, supporting meaningful experiences and preserving patient dignity.
History:
Heartland is division of HCR ManorCare, the leading provider of short-term medical care and rehabilitation for patients for patients who have been in the hospital as well as for those in need of long-term skilled nursing care.
It is a member of the National Association for Home Care and the National Hospice and Palliative Care Organization.
