Masonicare Partners Home Health & Hospice is a non-profit, state licensed, Medicare certified, JCAHO accredited Home Health Agency that offers a full array of home health and hospice services to patients and families through out the Hartford county. The agency provides multiple disciplines including: skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, home health aide and homemaker services to patients and families in their home 24 hours a day/7 days a week. The agency has specialty programs including Telemonitoring, Hospice, Palliative Care, Psychiatric Nursing, Infusion Therapy, and a Joint Replacement team.

History:
Masonicare Partners Home Health & Hospice is a merger of the E. Hartford branch of the CTVNA and the St. Francis Hospital Home Health Agency. The agency is non profit and owned in part by CTVNA and in part by St. Francis Hospital and Medical Center.
