The Farmington Valley Visiting Nurse Association proudly celebrated its 100th anniversary of community service in 2008. For a century, the FV-VNA has provided home health care, hospice services and preventive health programs to community residents. The Agency's mission is to provide and facilitate home and community-based acute, maintenance and preventative health services within its service area. Services are provided to any individual regardless of ability to pay, within its financial resources.
Description:
The Farmington Valley Visiting Nurse Association provides a comprehensive program of services, based upon the individual needs of the patient and family. Services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social worker services, nutritional counseling, home health aides, private duty aides, homemakers and companions. The Agency is a Medicare certified and state licensed organization which provides home health care, as well as hospice services and community health programs. Hospice is a specialized program of compassionate care which assists individuals and families who are coping with terminal illness. A specially trained team of caregivers coordinate and deliver care in order to maximize the quality of life. Community health programs are offered throughout the community in order to prevent illness and educate residents on healthy lifestyles. Clinics offered include blood pressure screenings, cholesterol checks, foot care, flu and pneumonia clinics, ear irrigation, healthy walking programs, vision and hearing screenings, and workplace education. A food bank is also operated in Granby, CT. Services are provided to residents in the towns of Avon, Bloomfield, Canton, North Canton, Collinsville, Farmington, Unionville, Granby, East Granby, North Granby, West Granby, Hartland, East Hartland, West Hartland, Simsbury, West Simsbury, Tariffville, Weatogue and West Hartford.
History:
The Farmington Valley Visiting Nurse Association has served the local community since 1908. Our roots began with community health and especially the care of newborns. The Agency, over time, has evolved into an organization with approximately 85 employees, providing over 26,000 home visits in the past year to residents. The Farmington Valley VNA also has local satellite offices in Granby, Farmington and Canton in order to retain a community based presence and convenient locations for health clinics and other illness prevention programs.
