Living Well Facilitators, or Lay Leaders, lead small groups of people with chronic diseases such as diabetes, cardiac disease, or COPD who are learning new ways for healthy living. By taking part in a Living Well Facilitator program, you will be able to: People with chronic disease themselves or who are caring for others with chronic disease are encouraged to attend!
*There is a volunteer commitment of providing two 6 week/2.5 hour Living Well classes in a year. Training Details: This is a free four-day training and all materials, breakfast, and lunch are provided each day.
