Crotched Mountain Community Care assists elders and adults with chronic illness and disabilities remain living in their own homes where they want to be by providing care coordination, advocacy and promoting independence.

Description:
We provide quality care management services to elders and adults with chronic illness and disabilities who are on the Home and Community Based Care program through Medicaid and we provide private pay care management on a sliding fee scale throughout New Hampshire

History:
Crotched Mountain Community Care, an affiliate of Crotched Mountain Foundation has been providing care management since 1986.
