Heart of America Hospice exists to support and provide symptom control to patients with a terminal illness through the use of an Interdisciplinary Team. Heart of America Hospice recognizes dying as part of the normal process of living and focuses on maintaining to quality of remaining life.

Description:
We are a Hospice agency taking care of patients of all ages. We serve them in their personal homes,Assisted Living facilities, Long term care facilities and hospitals. Our team consists of a Nurse Case Manager, Social Worker, Chaplain, Home health Aide,and Volunteer. Our main emphasis is the patient's physical comfort,but we also address psychosocial and spiritual needs of our patients and families.

History:
HOAH began in 1996, here in Topeka. We also have an office in Lawrence. Our service area continues to grow.
