RSS XML FeedBackground Active Apr 15, 2009 to Jun 19, 2009

Living Well Facilitators, or Lay Leaders, lead small groups of people with chronic diseases such as diabetes, cardiac disease, or COPD who are learning new ways for healthy living. By taking part in a Living Well Facilitator program, you will be able to:
-help others set goals that are do-able,
-learn to work with others,
-find support and solutions to address concerns,
-make daily tasks easier,
-relax and manage stress, and
-work in a partnership with the health care team.

People with chronic disease themselves or who are caring for others with chronic disease are encouraged to attend!

*There is a volunteer commitment of providing two 6 week/2.5 hour Living Well classes in a year.
