Description:
Applegate Hospice provides end-of-life hospice services for terminally ill patients in their place of residence, which can be in the home, facility or hospital setting. Each patient is assigned a care team which consists of a physician, nurse, certified nursing assistant, social worker, chaplain, and trained volunteers. Aside from nursing services, patients receive pyscho-social, spiritual and emotional services. Bereavement services are also available to family members up to 18 months after the patient passes away. Applegate Hospice is Medicare certified and accepts Medicaid and other insurances.

History:
Allen and Dixie Day, founders, started a long-term care facility in 1975 which was the start of Daywest Healthcare. Applegate was established in 1987 by Dan Hull and purchased by Daywest in 1999 because the Day family wanted to continue providing care to their patients in their homes. In 2002, Applegate started providing hospice services to also meet their clients' end-of-life needs.
