Our misson is to help elders and disabled live in their own homes safely and independently, expend their overall well-being and avert or delay nursing home placement.
Description:
The Living at Home/Block Nurse Program is a community based program that draws upon the professional and volunteer services of local residents. Services provided by volunteers include respite and family support, corresponding assistance, friendly visiting, shopping, volunteer transportation, escort transportation, monthly blood pressure and blood pressure checks, meal delivery, health education, light housekeeping and chores, information and referral, advocacy and Caregiver's Support Group.
History:
The Living at Home/Block Nurse Program was founded in 1999 as a 501 C (3) non-profit, the program was established to meet the needs of elders that live in their own homes. By enhancing and protecting the health and well being of elders, families and caregivers in our community, our program provides a means for people to improve and maintain wellness, stay in their homes they love, avert or delay nursing home placement and effectively prevent health care problems before they escalate. Our population is 3,038 in the community of Granite Falls (670 of those are seniors 65 and older) and we serve parts of Yellow Medicine and Chippewa counties. Of these people served, 49 of them receive direct care from nursing. There are currently 42 Living at Home/Block Nurse Programs in the state of Minnesota.
