Person-centered planning is a way to help make your dreams come true. In good person-centered planning, you invite family and friends to meet with you. A facilitator leads the planning in a way that honors your dreams and choices. Everyone agrees to support your dreams and an action steps are planned for making your dreams come true. |

Personal Plan Facilitation is a service offered through the Developmental Disabilities Waiver Program. Through this service, you can get person-centered planning. Participants of Mi Via can also use The Arc's Personal Plan Facilitation. |
