Program name: Memorial Medical Center Heritage Program for Senior Adults
Description : The Heritage Program for Senior Adults provides assistance with activities of daily living in a supervised setting while educating patients, families, and caregivers regarding mental health issues. Treatment is supervised by a psychiatrist and provided by a multidisciplinary team.
Hours of Operation: Monday-Friday 8:30am-5:00pm
Eligibility Requirements: This is a Medicare based program. Addition of a secondary insurance is needed. Medicaid also accepted. Patients under 65 years of age assessed on an individual basis.
Intake Procedure: Physicians, health professionals, family and self may refer patients to the program. Heritage Program provides a clinical evaluation at no cost to determine the need for treatment. Admission to the program will be offered only to those persons who can benefit from the program. To refer a patient please contact heritage program at (575)556-5910
