Description : The ACT program is designed to work with those individuals that need more intense services to prevent deterioration of mental health and/or substance abuse issues and to increase consumer's ability to function in various aspects of their lives. This program has a small consumer to staff ratio, as well as a variety of staff expertise to assist in these endeavors.

Eligibility Requirements: Serious mental illness with extensive hospitalizations and other services.
