Case Management
Services provided to children and families who are at risk for disruption of the family unit, children with serious emotional disturbance, or who are transitioning from hospitalization or residential treatment to outpatient care. Services are focused on providing structured interventions, in the home, to assist children/families in improved functioning of behaviors.

Monday-Thursday: 8:30 AM - 5:30 PM; Fridays 8:30 AM - 4:30 PM. Evenings and weekends as needed. |
| Eligibility Requirements: |
|
Children, ages 3-18, who meet criteria for serious emotional disturbance,
neurobiological disorder or behavior disorder. Services for Medicaid clients. |
