To promote the independence of older adults by directly providing services (Meals on Wheels, Homemaker services,general and High Risk Case management) or linking the individual to available resources within the community. |

Criteria is based on need and must live within city limits.
Client must be 60 yrs and older and have a disability (temporary or long term) that prevents them from performing most of their Activities of Daily living skills. |
