The Human Services Department's Medical Assistance Division (MAD) is the direct administrator of the New Mexico Medicaid program. Currently, there are approximately 40 categories of eligibility within New Mexico Medicaid. Some of these include Salud!, New MexiKids & New MexiTeens, Family Planning and Pregnancy, the Coordination of Long-Term Services (CoLTS) program, The Breast and Cervical Cancer Program and the Working Disabled Individuals (WDI) program. MAD is responsible for all Fee for Service (FFS) provider payments as well. |

| Intake Procedure: |
|
Applying for Medicaid is easy. You may download an application on the Medicaid website (http://www.hsd.state.nm.us/mad/), call 1-888-997-2583 to have an application sent to you or you may visit your local Income Support Division (ISD) office. Many clinics, hospitals, primary clinics and some school or community organizations can also assist with the application process.
Regardless of where you apply, you should take these documents with you when you go:
• 1. Proof of past four weeks of your family's income.
• 2. Social Security numbers for all applicants.
• 3. Proof of Citizenship for all applicants.
• 4. Proof of Identity for all applicants.
• 5. Proof of any other health insurance you may have.
Coverage for most Medicaid programs must be renewed every 12 months. Recipients must notify the State of New Mexico when they have a change in income or address. Medicaid covered services include preventative and specialty care. |
