Meals On Wheels By Senior Outreach Services

We maintain health and wellness safety nets for aging residents in our community. The services we provide are structured to assist seniors to live at home in safety, in comfort, and with dignity for as long as they can.

Description:
Meals On Wheels by Senior Outreach Services is comprised of 6 programs. The service area covered by each program varies.

• Meals On Wheels serves hot,nutritous meals to homebound seniors in the cities of: Antioch, Concord, Danville, Lafayette, Martinez, Moraga, Orinda, Pleasant Hill, Pittsburg, San Ramon and Walnut Creek.
  
  
• C.C. Café serves hot and nutritious meals at the following 6 locations: BayPoint (Ambrose Center), Concord (Concord Senior Center), Crocket, Pittsburg (Pittsburg Center Center), Rodeo, Walnut Creek (Walnut Creek Senior Center).
  
  
• Friendly Visitors serves all of Contra Costa County.
  
  
• Care Management serves all of Contra Costa County.
  
  
• Home Chore Registry serves all of Contra Costa County.
• Fall Prevention serves all of Contra Costa County.
  
  

Our clients are primarily older adults ages 60+ choosing to live at home as they age. Many are low-income and frail or disabled. Forty-five percent are ages 75-84. Twenty percent are age 85 and older – the fastest growing group in our community.

Meals On Wheels by Senior Outreach Services is an entrepreneurial nonprofit organization with over a 40-year tradition of excellence and service in Contra Costa County. Our success and expertise qualify us to manage community-enhancing health and human service programs for seniors. We enrich the lives of more than 5,000 county residents each year.
