Guardian Adult Day Health Care

Day care for the elderly and people with dementia, Alzheimer's, and any over 18 with disabilities, providing transportation, activities, meal and rehabilitation.

Description:
Medical Staff monitor daily medication and overall health maintenance as well as individual health concerns.
Social Services provide group and individual counseling concerning issues associated with aging. Referrals may be made to appropriate social service agencies.
Therapists provide an individual plan of care to assist participants with activities of daily living. Physical, occupational, and speech therapy groups are offered to maintain a participant's level of functioning.
Recreational Activities provide stimulating and structured programs, including exercise, art, movement, discussions, games, educational lectures, and cultural activities.
Nutritious Meals and Snacks are provided to promote better health and enjoyment of life's activities.
Transportation includes door to door service from the participant's home to the Center

History:
Guardian Adult Day Health Care Center was founded in 1994 to promote the well-being of elders and disabled individuals who live in Western Contra Costa County, alone or with others, in order to maintain them at, or restore them to, their optimal functioning potential and to allow them to remain in the community.
