Community Care - Fond Du Lac
Mission: Provide long-term comprehensive health and social services care to the elderly in a manner which enables them to remain safely in the community. CCE works to prevent the premature institutionalization or the unnecessary institutionalization of physically disabled adults and the elderly.
Description:
We currently have over 1000 participants enrolled in our PACE (Program of All Inclusive Care for the Elderly)and Wisconsin Partnership locations. We have 4 locations available to volunteer, they are: 1555 South Layton Blvd, 1825 N. Prospect Avenue, 5228 W. Fond du Lac Avenue and 3220 W. Vliet Street. Most of our volunteers help with activities such as arts and crafts, outings, music games, special events, etc. Other needs include one on one interaction. Bilingual volunteers are welcome!
