CentraCare Strives to enhance the quality of life for frail older adults residing in Calhoun County. Through unique partnerships, we seek to provide exceptional comprehensive and cost-effective services. Our interdisciplinary team strives to preserve individual autonomy, achieve excellent health outcomes and enhance the abiltiy of in formal caregivers to provide care to participants at home and within the community.
Description:
CentraCare is for those older adults who want to live in their own homes, but need some assistance to do so. They must be 55 years or older, live within Calhoun County and meet nursing home level of care. Through its day health center, which may be visited one to several days a week, supported by a variety of any necessary outside services, CentraCare helps keep its participants in the community. The day health center is open Monday through Friday from 7:00a.m. until 5:00p.m. The day health center provides the following services: recreation and social activities, meals for breakfast and lunch, primary care physician appointments, therapy appointments and specialty appointments. Transportation to and from home to the day health center and medical appointments.
