Home and Hospice Advantage provides superior end of life care driven by patients' choices and the manner that they wish to live out their lives. We believe in "quality in living". Our approach to hospice care creates a patient and family environment of hope, comfort and dignity. This approach is extended through our volunteers who can make an enormous difference in someone's life.

History:
Home Hospice Advantage is a Medicare and Medicaid certified program founded in 2004. We are an interdisciplinary team. In coordination with the physician we provide: specialized nurses, counselors and therapists, social workers, home care aides, volunteers, chaplains and bereavement counselors.
