Description:
Visiting Nurse & Hospice Care of Southwestern CT., Inc. will be recognized as the home health care and hospice provider of choice, as measured objectively by customer satisfaction, clinical outcomes, community health, financial stability and cost effectiveness. VNHC of Southwestern CT is committed to improving the quality of life for residents of the community through direct provision of services, case management, advocacy and education. PATIENT/FAMILY- CENTERED SERVICES ARE PROVIDED FOR ACUTE MEDICAL SURGICAL ISSUES TO MAINTENANCE OF CHRONIC CONDITIONS TO PALLIATIVE AND END-OF-LIFE-CARE. Volunteers participate in an extensive training program to prepare them to work with hospice patients and family members in home, hospital and hospice residence settings. Volunteers also assist the fundraising and office staff.
History:
Hospice in Stamford originated in the late 1970's when Jane Judd of the First Presbyterian Church/Stamford put together a small Planning Committee after hearing a presentation from Connecticut Hospice, the first Hospice in the United States. In January of 1980 the Stamford Foundation awarded the planning committee a $5,000 grant to continue community education and to train volunteers.In March 1980, the Rotary Club raised $12,000 to develop a home care program. Janice Casey (later President and CEO) was hired as the first hospice nurse. In February 1981, Hospice signed on its first patient/family. In 1998, Hospice Care merged with VNA Care, Inc. (a Visiting Nurse agency that had been active in the community for more than 85 years), becoming Visiting Nurse & Hospice Care of Southwestern CT.
