Screeners have a very important role at the clinic. As the screener, you will be the first person the patient meets. It is important to be professional and welcoming. Screeners interview new patients in the Walk-In Sessions (Adult Walk-In on Tuesday afternoons, Wednesday afternoons and Saturday mornings). The screener completes a form with the patient's demographic information and has the patient sign the Consent form. Confidentiality is very important. Accuracy and legible handwriting/printing are very important, as is confidentiality. Must be at least 21 years of age. Must be fluent in English and Spanish. Merritt Parkway from New York:
Exit 39A to the Route 7 Connector southbound. Take the Maritime Center/South Norwalk exit and turn right at the end of the ramp onto West Avenue. Follow signs for the Maritime Center to the 4th traffic light. Turn left onto North Main Street and continue ½ mile, passing under the railroad trestle where North Main becomes South Main Street. South Norwalk Community Center is on the left a few blocks down. Parking is available opposite and behind the building.
Merrit Parkway from New Haven:
Take Exit 40A. At the bottom of the ramp turn right onto Route 7, Main Street. Travel south 1 mile to the intersection with Route 123. Turn right and just after the bridge turn left onto the Route 7 Connector. Follow directions above.
I-95 from New Haven:
Exit 15. Take the ramp to the Maritime Center/South Norwalk. Turn left onto West Avenue and continue as above, turning left onto North Main Street after 5 lights. Continue as above.
I-95 from New York:
Take Exit 14. Turn right onto Fairfield Avenue. Continue straight at the traffic light at the bottom of the hill. At the railroad trestle, turn right onto South Main Street. South Norwalk Community Center is a few blocks down on the left. Parking is opposite and behind the building.
