The mission of the Easter Seals Blake Foundation (ESBF) is to enable each individual served to discover and meet his or her maximum potential for independent, productive living and developmental growth.

Description:
Founded in 1950, ESBF has been serving individuals with disabilities and their families throughout Southern Arizona, with the capacity to positively impact the lives of over 1.3 million people each year. We currently have a presence and offer programs in Pima, Cochise, Santa Cruz, Graham, Pinal and Greenlee counties.

Every dollar raised for the organization goes back into our local community.

In March 2007, we became the Easter Seals affiliate for Southern AZ.

Easter Seals Blake Foundation serves children, adults and families through Southern Arizona. Our programs engage a variety of clients ranging from children, to families, to adults with developmental disabilities.

Children & Family Services
Provides services to families and their children in seven southern Arizona counties. Programs provide support in the areas of health, parenting, child abuse and neglect, child development and early education, mental health and disabilities.

Services include:
• Family Support
• Family Preservation and Reunification
• Evaluation and Assessment
• Service Coordination/Family Resource Coordination/Support Coordination
• Training and Technical Assistance (offered in fourteen Arizona counties)
• Early Care and Education

Community Living Services (CLS)
Provides services to individuals with developmental disabilities, individuals with traumatic brain injuries and individuals diagnosed with a major mental illness. Services are designed to maximize the function of residents and are incorporated into the routine of the home where residents participate in the household task that needs to be performed such as cooking, laundry, shopping or house cleaning. CLS operates twenty-six homes in Tucson, Nogales, Sierra Vista and Safford, serving over 70 individuals ranging from 12-80 years of age.

• Individualized Habilitation
• Behavior Intervention
• Sensory Motor Development
• Rehabilitation Programs

SAGE Employment & Community Services
Provides services to individuals who are in their late teens through adulthood and operates with a philosophy based on the belief that children and adults with disabilities must be full and active participants in school, work and community occupying socially valued roles. Programs are offered in Tucson and Safford.

Services Include:
• Job Development and Placement
• Career Planning
• Supported Employment
• Personal Futures Planning
• Transition Planning (school to adult life)
• Independent Living Skills
• Leisure and Recreational Activities
• Community Involvement
• Respite
• Attendant Care
