Description:
Horizon Health Center was created in 1963 Founded in a small church basement Horizon eventually grew to become a full service Health and Community services organization. Presently Horizon Health Center provides care to over 14,000 unique recipients annually with an additional 3500 unique users of its community support services.

Founded in 1963 in a small church basement Horizon eventually grew to become a full service Health and Community services organization, becoming a fully accredited Federally Qualified Health Center (FQHC) in 1998 and a Joint Commission Accrediting Hospital Organization (JCAHO) certified two years later.
