Description:
Our care approach is patient-centered and involves an integrated program of therapies that address the total well-being of our patients. For each resident, we create an individual care plan and assemble a care team that may include specialists from several areas. The goal is to design a course of treatment and therapy that best meets the needs of the resident.

Celebrating life!
At Willow Creek Rehabilitation & Care Center, we look for ways to bring people together to share pleasant activities. Birthday parties, holiday parties, weekly bingo games and our annual family barbecue are just some of the many celebrations and special events we host for residents and families.

Special Outings
Residents at Willow Creek have many opportunities for outings and field trips in our wheelchair-accessible van. Several times a month we sponsor trips that include recreation, dining, shopping, sporting events or just enjoying a change of scenery. Our schedule includes movies, community events, garden shows and more.
Services include: Long-term care, Short-term care, Respite care and Hospice care.

History:
Willow Creek Rehabilitation & Care Center is one of seven nursing homes in New Jersey recently acquired by CPL Long Term Care. The centers were formerly part of Brandywine Senior Care. Willow Creek Rehabilitation & Care Center is a 180-bed skilled nursing facility providing short- and long-term care services.

“We are very pleased and excited about joining the health care community here in Somerset, “ said Stuart D. Lindeman, senior vice president of CPL. “And we’re very gratified by the warm welcome we’ve received from everyone.”

With the addition of Willow Creek Rehabilitation & Care Center and the six other Brandywine Senior Care centers, CPL now manages 29 nursing homes in the U.S.

According to Lindeman, the new management team plans few initial changes at Willow Creek Rehabilitation & Care Center. “This is a very good center,“ he said. “We’ve met with the current administrative team and with many staff members, and we are impressed with their dedication and skill. Our main priority will be to improve operational and management systems so the staff here can concentrate on delivering the best care possible to the residents.”

Willow Creek Rehabilitation & Care Center will continue to offer a wide range of health care services, including skilled nursing care, subacute care, respite and hospice care, and both inpatient and out patient rehabilitation services.
