MonmouthCares is a Unified Care Management Organization contracted to the NJ Division of Child Behavioral Health Services (DCBHS).

We work with children that have emotional or behavioral challenges and their families that are referred by DCBHS. We use a "wraparound" model to helps families create "Child and Family Teams" to support and empower them to move forward in their lives.

Wraparound i s process that improves the lives of children and families that have multiple needs.

Wraparound is a process that is child focused and family centered.

Wraparound helps to make a plan of care based on the strengths of the child and family.

Wrapround looks to the supports and services in the child's community to help the family make connections.

Wraparound respects who the family is. The culture and values of the family are important and guide how support is offered to the family.

Wraparound helps families build "Child and Family Teams to help them plan for their child and put the plan into action.

Wraparound is a process about the family's vision for an improved future.

Caring community members are critical to support families during times of crisis and transition and to help families have the skills and tools to move forward in their lives.

History:
MonmouthCares is one of the fifteen "Systems of Care" in New Jersey covering all 21 counties. Monmouth was one of the first three Care Management Organizations in New Jersey formed in 2001.

In 2009 MonmouthCares became one of the first three Unified Care Management Organizations combining the functions of Youth Case Management and Care Management. Children with moderate to intense needs are referred to us for care management services.
